As part of the grant acknowledgement process and to support the goals of the UPS Foundation, please schedule a meeting and/or public relations event with your UPS District Human Resources Manager, as provided by CIC. Please complete and return this form to document the involvement of UPS district employees in recognizing the CIC/UPS Scholarship Grant Program and its important contribution to making private higher education affordable and accessible.

Instructions for Completing the Form:
  1. Sign in.
    For proper tracking and for security reasons, accessing the form requires authentication. To view the form, please use the Sign In link at the top of the page to sign in with your username (email address) and CIC password.
    If you do not know your CIC password, you can retrieve your password online (If you do not have an account, you will be prompted to create one).

  2. Prepare attachments.
    Please provide CIC with any photos, press releases, or other publications that feature your State Council’s event with a UPS District Office.

  3. Complete form.
    Attach your materials, complete the form fields below, and click submit.
