Instructions for Completing the Form:

  1. For proper tracking of applications and for security reasons, accessing the application form requires authentication. To review the application form and to submit an application, please use the Sign In link at the top of the page to sign in with your username (institutional email address) and CIC password. If you do not know your CIC password, you can retrieve your password online (If you do not have an account, you will be prompted to create one).

  2. Next, please be prepared to submit a single package consisting of the following: (1) a one- to two-page project narrative (consisting of a brief overview of your current situation, a description of your current fundraising program, a description of the Challenge Grant goal and the activities planned to solicit matching funds, a statement of how member institutions will benefit from and award the student scholarships, and a statement of the State Council’s willingness to share at future State Councils gatherings what has been learned); (2) one-page project timeline; and (3) a letter of support from the chairperson of the State Council’s board of directors.

  3. Attach your application package, complete the form fields, and click submit.
