Instructions for Completing the Form:
  1. For proper tracking of nominations and for security reasons, accessing the nomination form requires authentication. To review the nomination form and to submit a nomination, please use the Sign In link at the top of the page to sign in with your username (institutional email address) and CIC password. If you do not know your CIC password, you can retrieve your password online (If you do not have an account, you will be prompted to create one).

  2. Next, please be prepared to submit a nomination package (one combined PDF document) consisting of the following:
    1. Nomination letter from the president or a vice president;
    2. Nominee's curriculum vitae; and
    3. Nominee's statement explaining his or her interest in and potential for a senior administrative position in higher education.
