Instructions for Completing the Form:

  1. Sign in.
    For proper tracking and for security reasons, accessing the application form requires authentication. To view the form, please use the Sign In link at the top of the page to sign in with your username (email address) and CIC password. If you do not know your CIC password, you can retrieve your password online (If you do not have an account, you will be prompted to create one after entering your email address).

  2. Review program information and Guidelines for Applicants.
    Please review the program information and Guidelines for Applicants before completing the application form, as these instructions do not include all the pertinent details for each question.

  3. Prepare attachments.
    Be prepared to submit an application packet (Word or PDF) consisting of the following:
    1. A signed letter of support from the president
    2. An application narrative of no more than five pages (2,000 words) that addresses the following questions:
      • What is the institution’s rationale for participating in the seminar, and what are the expected outcomes?
      • Which faculty members will participate?
      • How will the institution measure key outcomes?
      • How will the institution ensure that the new teaching methods are implemented during the grant period and continue to be applied afterward (that is, after the 2021–2022 academic year)?  
      • What plans does the institution have to expand the program and/or build upon positive outcomes beyond the grant period (that is, after the 2021–2022 academic year)?
      • What additional costs do you anticipate for the institution to participate in the seminar and then implement the new teaching methods (that is, costs not covered by CIC and the Keck Foundation)? How will the institution cover these costs?
    3. CVs of the faculty team members, including contact information (phone and email) and current teaching assignments, with a cover sheet describing why these faculty members were included.

  4. Complete form.
