Instructions for Completing the Form:
  1. For proper tracking of applications and for security reasons, accessing the application form requires authentication. To review the application form or to submit an application, please use the Sign In link at the top of this page to sign in with your username (institutional email address) and CIC password. If you do not know your CIC password, you can retrieve your password online (and if you do not have an account, you will be prompted to create one).

  2. Please review the Institute website and PDFGuidelines for ApplicantsOpens in new window before completing the application form, as these instructions do not include all the considerations that applicants should keep in mind.

  3. Please submit one application for one four-person team from the same institution. The team should include two faculty members, a senior academic administrator, and a second administrator who can take a lead role in bridging classrooms, student support services, and co-curricular activities.

  4. Be prepared to submit an application packet in Word or PDF (a single PDF file is preferred), consisting of the following:
    1. A signed cover letter from the president indicating the institution’s support of the application and commitment to the Institute’s specific goals and expectations.
    2. A brief biography of each team member.
    3. A narrative statement of no more than six (6) pages addressing: specific challenges or opportunities related to diversity and civility that the Institute may help address; previous or current efforts to support diversity and civility on campus; the relevant experience and campus roles of the proposed team members; and a preliminary plan to apply the content of the Institute to the curriculum and co-curricular activities.     4. Supplemental documents such as descriptions of current campus initiatives to support diversity and civility may be submitted but are not required.

  5. Attach your application materials, complete the form fields below, and click submit.
